Windows issue

hi!
i recently came into a problem at my parents computer and i have no idea how to solve it. i don't even know where to start from.
every file (be it mp3, avi, jpeg etc.) sort of duplicates itself into another folder-looking icon of 45kb. for example DSC00234.jpeg has next to it a DSC00234 folder, with nothing in it of course. i keep deleting them but they keep reappearing almost immediately. some, very few, i can't even delete anymore but i don't care about that, i just want to keep them away.
any helpful ideas, pls?

i have some bit defender running but he didn't find any viruses there. take into consideration also that the computer has never been connected to the internet either...
i will dld this stuff and try it.

Taskbar is mongoing. I mean the context menu. I can always show it (with right-click), but sometimes I can't click its items. Also the appearance of the context menu changes randomly. For example sometimes different item is written in bold, and sometimes "Maximize" is missing the 'e'. Here's some pics:
