ez horizontal. just grab one end, lift and drag basically. vertical would require some like grab, twist your hand or sach and would feel totally unnatural.

vertical axis, otherwise the thicker side of the pillow will end up in my neck, which sucks ultimately.

well usually i am lucky enough that i have sach a big pillow, i get two headrests on one side, so don't need to flip it, but ez vertical.

Horizontal, obviously. Its easiest, you just pull the top down. That way you get the thick chunk at the bottom too, all ready to squish back down with your head. Good stuff.
